Solera Health partners with health plans to offer programs designed to help members live healthier lives, such as the Diabetes Prevention Program (DPP). Solera helps identify what programs are best for you based on your current health plan status and preferences.
Also known as the DPP, the Diabetes Prevention Program helps participants lose weight, adopt healthy habits and significantly decrease their risk of developing type 2 diabetes. The program meets weekly for 16 weeks and then monthly for the balance of a year. The program teaches participants to make lasting changes by eating healthier, increasing physical activity and managing the challenges that come with lifestyle change.
This program is at no additional cost* to members who qualify. Once you enroll in the program, the health plan may receive a claim from Solera to cover the processing and program services for this preventive benefit. You may receive an Explanation of Benefits (EOB) from your health plan for this benefit. No action is necessary if you receive an EOB.
Everyone is different, but most participants say they spend 2 hours or less each week on education and tracking in their program. The programs include user-friendly tools that can be accessed anytime, anywhere – making it possible to spend less time as you get used to the program. For instance, online programs will include a scale that automatically syncs with the app for seamless progress tracking.
Once you enroll, you gain access to the program’s tools and support for a year. Program delivery varies, but they typically occur in phases. The initial phases (up to 16 weeks) have more activities to provide the education and support for getting started. The following phases focus on maintaining the health habits you’ve established. We strongly encourage participants to utilize the tools throughout the year.
